Client Intake Process
Step 1: Schedule AN Intake Phone Appointment
Before scheduling an appointment in our office or an online Zoom appointment, the counselor needs to have a brief 15-minute phone appointment with you. This call is to evaluate whether your counseling need is something we can help with and to discuss our availability to meet your need.
If you would like to schedule this intake phone call, you can use the chat intake form below or call our office at 317.585.1845.
The Administrative Assistant will help you schedule the intake phone call.
Step 2: Talk to the Counselor
At the scheduled time of your phone appointment, you can call the office at 317.585.1845. Rev. Jeffrey Pokone will be expecting your call. During the phone appointment, you will need to share a brief explanation of your counseling need. This will allow him to assess your needs better and figure out how to help you. Please feel free to ask any questions you might have during this call.
When counseling women in the office, we highly recommend that they have a trustworthy support person attend their counseling sessions. This unique approach has helped them receive the care and support they need in their everyday lives.
Step 3: Schedule Your Initial Counseling appointment
Based on the counselor’s assessment of your needs and our availability to serve you, he may suggest one of the following:
Schedule the initial in-office or online Zoom appointment.
We begin with an initial one-hour counseling appointment before scheduling additional follow-up appointments.
Recommend a resource or provide immediate counsel.
Online clients must submit their new client forms by email before their first intitial appointment.
New client forms can be filled out online and emailed to the office before their first appointment. If you need to, you can print them and bring a hard copy to the office.
If you have any questions, please feel free to email, use the chat intake option, or call our office.
